They're attached by like a vein-cord if I remember correctly but are otherwise floating. Which after I researched that made me cry and convince myself it could happen to me.

At any rate when he took an axe to the scrotum itself and then cut open a hole I think it's possible that he could have severed that cord. At any rate I don't see what else he could have replaced it with in the video to make it look real.
