They're attached by like a vein-cord if I remember correctly but are otherwise floating. I dated an ex from Lebanon who had a genetic disease (I forget the name of it right now but it was something like Mediterranean fever) where one of the symptoms was his testicles could rotate within the scrotum causing incredible pain and potentially killing them altogether.
Which after I researched that made me cry and convince myself it could happen to me.
At any rate when he took an axe to the scrotum itself and then cut open a hole I think it's possible that he could have severed that cord. At any rate I don't see what else he could have replaced it with in the video to make it look real.
